South Haven Family Physicians understands that some patients may appreciate having the opportunity to fill out our patient forms in the comfort of their own homes, where they have the time to complete some of our more detailed sections at their own pace.

Our patient registration forms are available for download for those who feel more comfortable filling out the forms at home. Just bring them with for your appointment. If you’d prefer to complete the forms at the time of your office visit, that works just fine for us too.

Walk-in appointments:
W e always have same day appointments available for urgent needs. Just call ahead of time to schedule an appointment and we will make every attempt to accommodate your needs.

What if I need to see a doctor after hours?
For your convenience we offer evening appointments on Tuesdays & Wednesdays until 6:30 p.m. and on Thursdays until 7:00pm
SHFP also provide Saturday morning appointments from 10:00 a.m. to 12:00 p.m. for your urgent care needs; just call the office on Saturday morning (269) 637-1115 anytime after 9:00am.
Outside of our extended office hours, please contact us by telephone at (269) 637-5271. Your call will be directed to our after hours service who will assist you with your call by taking your contact information.  A South Haven Family Physicians  provider will return your call.

What if I need Emergency Care?
To provide adequately for your emergency medical needs, please contact us by telephone at (269) 637-5271. Your call will be directed to our after hours service who will assist you with your call and take your contact information. For life-threatening emergencies (severe difficulty breathing, chest pain, unconsciousness, seizures, or severe head injury) call 911 immediately.

What age range do your physicians treat?
South Haven Family Physicians provides care from newborns to geriatrics. We take pride in providing comprehensive services for every stage of life.

When is my child due for immunizations?
Using our Electronic Medical Records we assist you in keeping your children’s immunizations up to date and will mail you reminders

How do I get my lab test results?
You will be notified within two to three days after we receive your results. South Haven Family Physicians is proud to be one of the first family medicine providers in Southwest Michigan using an electronic medical records system. What that means for our patients is faster test results. Because of direct interfaces with local testing facilities, some lab test results can be received back next day.

Will my insurance pay for the services?
Your insurance card should indicate the name of the group you are insured with. Please refer to our list of contracted insurance providers to see if we are contracted with your insurance provider. South Haven Family Physicians advises patients to contact their insurance as each plan offers different levels of coverage. Please check the back of your wallet card for contact telephone numbers. If we are not contracted with your carrier, you may be responsible for a larger balance than you have experienced in the past.

Helpful things to bring on appointment day:
Please bring your insurance card, list of current medication and any co-pay you may have to your appointment. This helps us process your insurance more quickly and provide you better service.  Also, bring any forms you may have printed from this website and completed at home.

How can I get a prescription refilled?

Please call your pharmacist for prescription renewals. So you don’t run out, prescriptions and authorizations for renewals should be requested 2 to 5 days in advance.  If your re-fills have run out, please call and make an appointment.

What if I need to see a specialist?
As a primary care practice, we can take care of most of your health care needs, some care may require a referral to a specialist. Some insurance companies require you to have a formal referral in place before you make your appointment with the specialist.

How can I get copies of my Medical records?
By law, patients requesting their medical records must submit a written request. For your convenience a Medical Release form PDF* can be downloaded and submitted via fax or to the address included on the form. Please allow two to three weeks for medical record requests.

If you have an urgent request, it is recommended that you call our office at (269) 637-1115, and ask to speak to a medical records staff person. There is a minimum $25.00 fee for copying your medical records.  Download a Medical Release form.
