How can I get a prescription refilled?
Please call your pharmacist for prescription renewals. So you don’t
run out, prescriptions and authorizations for renewals should be requested 2 to 5 days in advance. If your re-fills
have run out, please call and make an appointment.
What if I need to see a specialist?
As a primary care practice, we can take care
of most of your health care needs, some care may require a referral to a specialist. Some insurance companies require you
to have a formal referral in place before you make your appointment with the specialist.
How can I get copies of my Medical records?
By law, patients requesting their medical records must submit a written request. For your convenience a Medical Release
form PDF* can be downloaded and submitted via fax or to the address included on the form. Please allow two to three weeks
for medical record requests.
If you have an urgent request, it is recommended that you call our office at (269) 637-1115, and ask to speak
to a medical records staff person. There is a minimum $25.00 fee for copying your medical records. Download
a Medical Release form.
